Group Home Care
Group homes are small, residential facilities located within a community and designed to serve children or adults with chronic disabilities. Our group home services have six or fewer occupants and are staffed 24 hours a day by trained caregivers. Our caregivers and registered nurses are here to help. Help with hospital discharge
You may be discharged from hospital to continue recovering from home when you no longer need a high level of hospital care. Healing at home is more comfortable and safer with the right support. You are in a familiar environment at home where risk of falls and infection is lower. However, now that you are getting better at home at your own pace, you should make a plan with your physician and family about the types of home and activities you will need help with.
